Job Description
Summary
The Department of Neurology is looking to recruit a Registered Nurse I to serve as the clinical care navigator for the Neuromuscular practice and ALS multidisciplinary clinic in the Texas Medical Center. The candidate will work directly with physician leadership, clinical management, and clinical support staff to serve the patients and their families. A successful candidate will manage complex patient cases, coordinate care plans, provide educational resources, and help patients navigate the treatment process.
Job Duties
- Manages care coordination, navigation, and treatment plans for complex patient cases.
- Schedules appointments and maintain a patient database to ensure quarterly appointments across multiple specialty and subspecialty providers.
- Effectively communicates with patients, families, care team providers, and clinical staff.
- Creates pre-clinic patient summary sheets, provide after-clinic summary sheets to the patient, and maintain documentation for the ALS clinic.
- Manages the medication process including prior authorization submissions, Tofersen coordination with the central pharmacy, appealing denials, and documentation.
- Answers patient calls/messages and direct concerns to appropriate team members or the Emergency Department.
- Manages EPIC in-basket messages, coordinates referrals, and enters orders for patient testing.
- Provides testing and testing support for Slow Vital Capacity (SVC), Forced Vital Capacity (FVC), and genetics.
- Coordinates, tracks, monitors, and orders supplies for Percutaneous Endoscopic Gastrostomy (PEG) tube placements.
- Organizes the pre-clinic team meeting including room reservation, clinic attendance communication, and patient care needs updates to the providers.
- Assists providers with lumbar punctures and Tofersen medication administration.
- Coordinates home health, palliative care, hospice care, and durable medical equipment needs for patients.
- Helps patients understand social security, disability, and Medicare services available for patients with a diagnosis of ALS.
- Partners with ALS Association for equipment needs that patient is unable to obtain with insurance (from ALSA loaner closet).
- Performs other duties as assigned.
Minimum Qualifications
- Associate's Degree or Bachelor's Degree in Nursing.
- No experience required.
- Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
- Basic Life Support (BLS) Certification from the American Heart Association.
Preferred Qualifications
- Bachelor's Degree in Nursing.
- Demonstrated experience with EPIC or a similar Electron Medical Records (EMR) system.
- Strong written and verbal communication skills.
- Ability to display a professional and positive attitude.
- Ability to work independently and collaboratively across the Department.
- Detail-oriented, self-motivated, efficient, and strong organizational skills.
