Pharmacist Foundation-Per Diem

This role works under the supervision of the Director of Pharmacy Clinical Network, and according to department policies, and procedures, and guidelines, is responsible for the compounding and dispensing of prescribed pharmaceuticals. Will be responsible for checking all medication prior to dispensing and deliveries. Responsible for enrollment and ongoing prescription adherence follow-up for all patients referred by pharmacy liaisons for Specialty Care Management. Responsible for comprehensive medication reconciliation with electronic medical record of each patient during prescription verification. Direct point of contact to provide pharmacy clinical support for all enrolled specialty clinics when needed.

***Please note, this per diem position will support and cover both Long Beach and South Pasadena Clinic sites***

As a successful candidate, you will:

  • Preparation and dispensing of medications in the retail and specialty pharmacy setting.
  • Maintenance and review of patient medication profiles.
  • Counseling of patients on appropriate medication management.
  • Review medication orders and patient medical records.
  • Providing high quality care and customer service to other health professionals.
  • Prevent inappropriate or misuse of medications.
  • Assisting with insurance processes.
  • Mentor new staff and consistently models service excellence.
  • Obtain and maintains licenses and certificates required for job function and completes all assigned trainings in a timely manner.
  • Follow established City of Hope and department policies, procedures, objectives, performance improvement, attendance, safety, environmental, and infection control guidelines, including adherence to the workplace Code of Conduct and Compliance Plan.
  • Practice a high level of integrity and honesty in maintaining confidentiality.
  • Promote collaboration and teamwork among all members of the healthcare team.
  • Maintain compliance with all rules and regulations governing the practice of Pharmacy and City of Hope policies and procedures.
  • Performs other duties as assigned by the Leadership Team.

Your qualifications should include:

  • Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
  • Current CA Pharmacist License
  • BLS through the American Heart Association

Additional Information:

  • Hourly Per Diem Rate: $88.84
